Post-Conference Activities |
Purpose
The purpose of this post-conference project is to leverage the momentum and enthusiasm generated by Faces of a Healthy Future: National Conference to End Health Disparities, in moving toward the goal of eliminating health disparities.

Goal One
Evaluate the effectiveness of the conference in facilitating participants to incorporate conference information into their health disparities projects and developing collaborations and partnerships
Conference Evaluations Conducted at the End of The Conference, One-Month, Six-Month and Twelve-Month Intervals Will Include:
- Immediate data analysis, compilation and presentation of participant evaluations related to the relevance to their projects
- Evaluations of participants' integration of conference information into efforts to eliminate health disparities
- Assessment of the effectiveness of collaborations and partnerships forged as a result of the conference
- Assessment of the participants’ post-conference perspective of its significance in developing strategies to eliminate health disparities

Goal Two
Disseminate conference information, proceedings and evidence-based strategies for eliminating health disparities nationwide
Information Dissemination Will Include:
- Reports to local, state and national officials, participants, private organizations, Veterans associations, African American, Hispanic American, American Indian, and Asian American organizations, HBCUs, other academic institutions, health care providers and faith-based and community leaders
- Published documents that presents evidenced-based strategies discussed in presentations, posters, exhibits and group meetings
- Blogs, twitters, and community forums.

Goal Three
Continue the three preconference Strategy Planning Groups and the four National Community Working Groups.
Continuation of groups establised at the conference:
- Blogs, twitters, and community forums for the National Working Groups and the preconference Strategy
Planning Groups to continue collaboration as well as participant
communication.
